U.S. President Barack Obama delaying his visit in Indonesia as facing a vote in Congress (and Senate) on the Draft Law of Health Services, better known as the Health Reform Bill. In yesterday’s vote, the Act was finally passed, although special arrangements postponed until Obama’s abortion issue a presidential decree (executive order).
Obama’s attitude is concerned with the health reform this affair shows how the American political arena, health occupying an important place.
In almost every presidential campaign, if not wrong since Truman, health became one of the main topics in addition to employment opportunities and foreign policy. Actually the idea of health reform that would transform the health system is a bit drastic been proposed Hillary Clinton during her husband became president, but did not get enough support in Congress, especially since the majority of Congress controlled by Republicans who rejected the reform.
Core of the proposed health reform President Obama is not too far from Hillary Clinton’s first concept: providing health insurance for all Americans. Pembedanya is this Obama proposal would reduce the income of doctors and are designed to reduce its budget deficit by 132 billion dollars a year in the next ten years, but initially will suck a budget of 871 billion U.S. dollars to be gained by raising taxes for insurance premiums insurance high (above 8500 U.S. dollars per person per year).
In other words, funding is done by cross-subsidies. In addition, everyone should have health insurance, either paid or buying their own employer, under penalty of fines if it does not have.
Will be more expensive
It must be understood that the health budget in the U.S. is too high, nearly 19 percent of GDP, and is expected to continue to increase, which made the U.S. more and more people who would be unable to pay the cost of treatment when sick. Financing system is still based fee for services that are likely to be more expensive.
Health insurance held by the private sector, except for Medicaid (for the poor) and Medicare (for elderly) given the state based on the Social Security Act of 1965. However, there are approximately 40 million people without health insurance are protected.
They are no longer categorized as poor, but also can not afford private health insurance policy. Obama wants to overhaul it so that not a single American citizen who had health insurance protection. This is the core ideals of Obama’s health reform.
In the reform framework, each company must pay insurance premiums of its employees. Small and medium companies will be given a tax deduction incentive. Freelance workers told to buy their own insurance. To be able to afford premiums, Obama planned to form a pool (the stock) health insurance in every state. This concept was proposed Hillary Clinton. By placing an insurance company in such a pool, they will be able to reduce the cost of marketing and bureaucracy costs.
It is estimated that 30 percent of the price premium is spent on marketing and administrative costs of insurance companies. It contributed to the price of premiums to be expensive without improving the quality of service. In contrast to the good English and insurance services managed by the state, in the United States all handed over to the private sector. Obama apparently did not want to change the existing system.
Republican party is not not agree with the reforms, but they assume that the Obama proposal will not be able to walk because the people who raised taxes on the rich would have refused. They also bring a covered service, there termasukkah abortion services?, And that the funds should be provided will exceed one trillion U.S. dollars over 10 years.
Insurance companies would tend to support the opinion of the Republican Party. Some states are Republican governors have requested that there is a choice for each state, Obama would join concepts or not. Anyway with all the excuses, the Republicans seem to want to derail the concept of Obama.
Unfortunately, with the victory of a Republican Senator, Scott Brown, in Massachusetts (which won for decades Democrats), the position of Democrats in the Senate to shake. Moreover, the state of Massachusetts is already implementing a system like the concept of universal coverage Obama. These events can give the impression that the people of Massachusetts was not satisfied with the new system.
If the concept of Obama is rejected by the Senate despite Congress accepted, there will be adverse political dilemma Democratic Party in the upcoming November mid-term elections. Therefore, it is understandable why Obama chose to focus on Health Law and postpone his visit to Indonesia.
Situation in Indonesia
Indonesia has a similar situation with the United States, but shy to take a firm stand. Law of National Social Security System states that every citizen must take health insurance. Management of insurance submitted to the state-owned enterprises, however, also behave like a private. Service was a mix of government and private, even in the area: the government was behaving like a private.
It is unclear whether we will embrace the whole concept of British government handled either health insurance or services, or the U.S. system is entirely left to the private sector, except for the poor and elderly. How well do with local autonomy? Meanwhile, the cost of health services become less affordable and the quality of services was not guaranteed.
